［記者黃淑莉／雲林報導］中國醫藥大學北港附設醫院（北港媽祖醫院）心臟血管科「救心團隊」成功以「豬尾巴」導管將血栓攪碎，救回兩側肺動脈栓塞的72歲婦人。
72歲婦人今年2月突然呼吸不順，家人緊急將她送到北港媽祖醫院急診，到院時低血壓、意識不清、皮膚濕冷、呼吸短促，呈現休克狀態，電腦電層掃描發現兩側肺動脈栓塞，緊急插管治療並施打升壓劑、血栓溶解劑。
心臟內科醫師張志斌指出，緊急處置後婦人情況並未改善，由於婦人已呈休克狀態，傳統葉克膜輔助支持療法已來不及，救心團隊決定施行緊急心導管介入治療。
張志斌表示，以「豬尾巴」導管將血塊攪碎成小血塊後，再投以小劑量血栓溶解劑，可大幅增加藥效，讓患者脫離休克狀態，而且邊做心導管邊施換第2種強心劑維持血壓，經過一小時搶救，成功將婦人救回，住院2週後出院，經過2個月門診追蹤恢復狀況很好。
張志斌說，肺動脈栓塞大部分是深部靜脈栓塞的併發症，急性、大面積肺栓塞、休克的病人三個月死亡率高達58％，久坐、久站、久臥及服用口服避孕藥、癌症病人或外科手術後的病患都是高危險群，若有發生呼吸窘迫及以上危險因子，就要高度懷疑並到大醫院診療。
北港媽祖醫院院長吳錫金強調，儘管醫療方式、器械越趨多元，但醫師的專業判斷是關鍵，婦人發生原因不明，張醫師正確判讀病患情況、當機立斷選擇最適當的治療方式，把握搶救黃金時間，順利救回患者。
